Why Real Food Still Beats a Pill
For over 40 years in clinical practice, I have consistently found one food to be among the most powerful natural interventions available: garlic.
Not turmeric. Not green tea. Not resveratrol. Not any isolated phytochemical. Garlic.
If there were a single food I would place at the top of the medicinal food hierarchy, garlic would be at the top of the list. Modern science is now beginning to explain why.
A remarkable new study published in Cell Metabolism identified a compound derived from aged garlic that activates powerful anti-aging pathways, improves communication between fat tissue, the brain, and skeletal muscle, and significantly improves muscle function during aging. The findings suggest that compounds from garlic may influence the body’s aging process itself. But there is an important point that often gets missed.
Aged garlic is wonderful. Raw garlic may be even more powerful. The reason is simple:
Nature created garlic as a living food first — not as a supplement.
The Difference Between Raw Garlic and Aged Garlic
When a fresh garlic clove is crushed or chopped, an enzyme called alliinase converts alliin into one of the most biologically active compounds in nature: allicin. Allicin is responsible for much of garlic’s:
- Antibacterial activity
- Antiviral activity
- Antifungal activity
- Anti-inflammatory activity
- Immune-supporting activity
- Anticancer activity
The problem? Allicin is fragile. Heat destroys much of it. Processing reduces it. Manufacturing changes it. This is why raw garlic often produces stronger antimicrobial effects than cooked garlic or supplements. Fresh garlic is particularly rich in allicin and other reactive sulfur compounds that give garlic its characteristic smell and much of its biological power.
New Research Supports Raw Garlic
A comprehensive review published in Frontiers in Nutrition analyzed clinical and observational studies involving raw garlic consumption. Researchers reported associations between raw garlic intake and:
- Improved cholesterol levels
- Better blood pressure regulation
- Improved glucose metabolism
- Enhanced antioxidant status
- Improved fibrinolytic activity
- Better liver function
- Improved insulin regulation
- Reduced cardiovascular risk
- Reduced cancer risk markers
The authors concluded that raw garlic consumption is associated with broad improvements across multiple systems of the body. This is exactly what clinicians have observed for decades.
Garlic rarely works on only one system. It works on many systems simultaneously because it targets some of the most fundamental biological pathways of health.
Garlic and the Immune System
Another major review recently published examined garlic’s sulfur-containing compounds and their biological activities. Researchers documented that garlic-derived compounds influence:
- Oxidative stress
- Inflammation
- Immune regulation
- Bacterial growth
- Fungal growth
- Viral activity
- Cellular signaling pathways
These organosulfur compounds appear to regulate some of the same pathways involved in cardiovascular disease, aging, chronic inflammation, and cancer progression. This helps explain why garlic has maintained its reputation across thousands of years of human history.
Garlic and Cancer Research Continues to Expand
Beyond its well-known antimicrobial and cardiovascular benefits, garlic is now attracting attention for another reason: its potential influence on longevity pathways involving NAD+ metabolism and the sirtuin family of proteins, particularly SIRT1.
The new Cell Metabolism study found that a compound from aged garlic extract enhanced signaling involving extracellular nicotinamide phosphoribosyltransferase (eNAMPT), a key regulator of NAD+ biosynthesis. NAD+ is a critical molecule required for cellular energy production and for the activity of sirtuins, often referred to as “longevity proteins” because they help regulate DNA repair, mitochondrial function, inflammation, and metabolic resilience.
As NAD+ levels naturally decline with age, sirtuin activity tends to decline as well. Researchers observed that garlic-derived compounds appeared to support this signaling network, improving communication between adipose tissue, the hypothalamus, and skeletal muscle. The result was measurable improvement in muscle function during aging, suggesting that garlic may influence not only disease-related pathways but also some of the biological mechanisms associated with healthy aging.
What if one humble kitchen ingredient could influence cancer-related pathways at nearly every stage of the process — from abnormal cell growth to inflammation, angiogenesis, and metastasis? That possibility is one reason garlic has become one of the most intensely studied foods in cancer-prevention research.
A recent peer-reviewed review published in Nutrients summarized evidence showing that garlic and its organosulfur compounds may influence multiple mechanisms involved in cancer development and progression. The review discussed evidence related to:
- Induction of apoptosis (programmed cancer-cell death)
- Inhibition of tumor-cell proliferation
- Reduction of oxidative stress
- Modulation of inflammatory pathways
- Suppression of angiogenesis (new blood vessel formation that feeds tumors)
- Regulation of signaling pathways involved in metastasis
Researchers noted that compounds such as allicin, diallyl sulfide, diallyl disulfide, and S-allyl cysteine have demonstrated anticancer activity across a variety of experimental cancer models. The authors concluded that garlic remains one of the most extensively studied functional foods for cancer prevention research and supportive health strategies.
Additional Perspective on Garlic’s Long History
Garlic has been used medicinally by cultures throughout Egypt, Greece, Italy, China, India, and the Middle East for thousands of years. Historical records describe its use for supporting vitality, digestion, resistance to infections, and overall health. While traditional use alone does not prove effectiveness, it is remarkable that modern research continues to validate many of the benefits that ancient practitioners observed long before the development of modern laboratories.
Interestingly, the newer aging research suggests garlic may influence pathways linked to cellular energy production and longevity, including mechanisms involving NAD+ metabolism and the SIRT family of genes, particularly SIRT1. Sirtuins are often referred to as “longevity genes” because they help regulate DNA repair, mitochondrial function, inflammation, and metabolic health. The new study found that aged garlic compounds enhanced signaling involving eNAMPT, a key regulator of NAD+ production, which is closely connected to sirtuin activity. As NAD+ levels decline with age, sirtuin function also tends to decline, contributing to reduced muscle performance and metabolic resilience.
What makes the study especially noteworthy is that the benefits were not limited to laboratory markers. Researchers observed improved communication between adipose tissue, the hypothalamus, and skeletal muscle, resulting in measurable improvements in muscle function during aging. In other words, garlic was not simply affecting isolated cells — it appeared to improve whole-body signaling networks that help preserve strength and muscle performance as we grow older.
Many ask how garlic compares with turmeric. Both are exceptionally well-studied medicinal foods, but they tend to work through somewhat different mechanisms. Turmeric’s primary active compound, curcumin, is best known for its potent anti-inflammatory effects and its influence on pathways such as NF-?B, COX-2, and various inflammatory cytokines. Garlic, by contrast, has a broader body of evidence related to cardiovascular health, immune function, antimicrobial activity, metabolic regulation, and now potentially longevity signaling.
The New Anti-Aging Discovery
The newest breakthrough comes from the new Cell Metabolism study. Researchers discovered that S-1-propenyl-L-cysteine (S1PC), a bioactive compound found in aged garlic extract, activates a longevity-related pathway involving:
Fat Tissue ? Brain ? Muscle
The compound stimulated LKB1 signaling, increased secretion of eNAMPT, improved hypothalamic communication, and ultimately improved muscle function during aging. In simple terms, garlic appears to help different organs communicate more effectively as we age.
The result is improved muscle function, improved metabolic signaling, and potentially healthier aging. This is one of the most exciting anti-aging discoveries involving a common food in recent years.
Why Food Is Still Superior
The study used a compound derived from aged garlic extract. That is valuable. But we should remember an important principle:
Supplements are substitutes. Food is the original design.
Fresh garlic contains hundreds of naturally occurring compounds that work together in ways science still does not fully understand. When we isolate one compound, we may obtain some benefits.
When we consume the whole food, we often receive a broader range of compounds and effects. Real food remains the foundation of health.
Garlic and Cancer
One of the most exciting areas of garlic research remains cancer prevention and support. Recent reviews continue to demonstrate that garlic compounds influence:
- Cell-cycle regulation
- Apoptosis (programmed cell death)
- Oxidative stress
- Inflammatory signaling
- Tumor proliferation
- Metastatic pathways
Researchers have reported that garlic compounds can suppress cancer-cell growth while influencing multiple signaling pathways associated with tumor progression. This does not mean garlic is a cure for cancer. But it does mean garlic belongs among the most extensively researched anticancer foods in the world.
Practical Application
If tolerated, consider making fresh raw garlic part of your routine. A simple approach is:
- Crush put through a garlic press or finely chop one fresh clove to start. More is better start small and build up.
- Consume it with food. Especially with fats like avocados.
For those who cannot tolerate raw garlic, aged garlic extract represents an excellent alternative supported by modern research.
The one I found superior after comparing all of them on the market over many years is the only garlic supplement we have ever carried. Our garlic extract is as close as you can get to the real thing not being the real thing. All of our patients progressively increase their garlic intake.
Final Thoughts
The newest science is confirming what traditional healing systems have believed for centuries…
Garlic is not merely a seasoning. It is one of nature’s most powerful medicinal foods.
The new Cell Metabolism study shows that compounds from aged garlic can improve age-related muscle decline and activate longevity pathways. Yet the larger lesson may be even more important: the greatest pharmacy in the world is still the produce section.
Real food. Living food. Whole food.
And garlic remains one of the most compelling examples of how a common food can support health across multiple biological systems.
